Clinical prevention interventions, just like quality improvement interventions, are meant to help in boosting the safety and health outcomes of patients. One of the conditions prevention intervention can greatly reduce its impact is cardiovascular disease. Indeed cardiovascular disease is the number one condition that leads to deaths and disabilities globally, as it kills seventeen million people every year. One clinical prevention intervention for cardiovascular disease is the use of team-based care (Arnett et al., 2019). In the team-based care model, two or more health care providers collaboratively work with every patient. In preventing cardiovascular disease, multidisciplinary teamwork together towards educating patients about the disease, identify the cardiovascular disease risk factors, prescribe medications and do modifications where necessary, and maintain a close discussion with patients regarding their conditions and state of health. Such a multidisciplinary team can have various professionals such as community health workers, primary care providers, community paramedics, pharmacists, nurses, and doctors.

Multidisciplinary team-based clinical prevention intervention for cardiovascular disease has also been successful. A team-based approach is a method that is continually being embraced in health care settings to not only prevent diseases but also to manage and treat them. The use of a team-based approach has seen individuals identified at risk of getting cardiovascular disease work on ensuring that their blood pressure and weight, among other things, are checked and put under control (Arnett et al., 2019). High blood pressure and abnormal bodyweight have been identified as part of the cardiovascular disease risk factors. It is, therefore, not a surprise that multidisciplinary team for cardiovascular diseases usually target their control. The method has been successful since most healthcare facilities are willing to make collaborations and set up multidisciplinary teams to help prevent cardiovascular disease.

The main intervention you noted in controlling cardiovascular disease is using a team-based care model, which involves two or more healthcare workers working together. This is the right choice of intervention because it links different healthcare professionals who contribute to the wellbeing of the patients suffering from cardiovascular diseases. Felman (2019) discusses some of the prevention and treatment methods such as medication in reducing cholesterol levels for increased blood flow, surgery, physical exercise prescriptions, and dieting. All these prevention and treatment processes require specific professionals such as nutritionists, physiotherapists, clinicians, and educators. This is a suitable intervention since it includes bringing all professionals on board to control the risk factors and treat cardiovascular diseases.

In the article, you have not indicated some of the measures of success in implementing the initiative, such as shorter admission periods, reduced cases of cardiovascular diseases, and increased rate of healing.

In the initiative that you have proposed for intervening in cardiovascular diseases, you may also have a plan of care with different stages of intervention for every cardiovascular patient. For instance, a clinician may be involved in the control and treatment of the disease while the educationist may follow before the physiotherapist and dietician follow in the line of treatment and management. This plan may help in ensuring a procedural control of the disease. It is not good to bring all the professionals on board at the same time, and thus, the design needs to have a specific workflow to ensure the program’s effectiveness.
