Standard 3 – Outcomes Identification –

Week 7:

My objective (s) for these standards is/are to:

·         To increase my interpersonal communication skills for effective data collection for patient assessment.

·         To gain enough knowledge on differential diagnosis for accurate diagnosis

·         To learn how to identify the anticipated care outcomes in drawing the plan of care.

i.        Maintain a close relationship with the teammates and nurse leaders for effective mentorship.

ii.      To regularly preview diagnostic features of various diseases and regularly confirm with key resources for effective diagnostic process.

i.        I will effectively draw patient care plans in achieving the anticipated care outcomes.

i.        There is coordinated care in the organization where other healthcare providers help in confirming the diagnosis.

ii.      The organization’s nursing leaders provides guidance and reviews for nurses under the practicum program.

Standard 2 – Population Diagnosis and Priorities,

Standard 3 – Outcomes Identification – Week 8:

My objective(s) for these standards is/are to:

·         To improve my abilities in effectively collecting the required patient data on all common diseases for effective diagnosis.

·         To effectively diagnose all diseases.

i.        Maintain a close relationship with the teammates and nurse leaders for effective mentorship.

i.        I effectively take patient assessment data without missing details for diagnosis.

ii.      I will improve my accuracy in diagnosing diseases to at least 80%.

i.        There is coordinated care in the organization where other healthcare providers help in confirming the diagnosis.

ii.      The organization’s nursing leaders provides guidance and reviews for nurses under the practicum program.

The internship program on public health nursing practice is very essential in improving my skills, abilities, and exposure to nursing. In the agency, we perform activities such as making home visits for nursing care, providing nursing care to chronic illnesses, and conducting public health education programs to the patients and the general public. This has been an effective platform to improve nursing capacities. In the daily activities, I am required to make patient assessments for diagnosis and developing patient care plans for effective delivery of patient care. All these are related to the three nursing standards on assessment, diagnosis, and patient care outcome analysis. In achieving the weekly objectives, I have to effectively coordinate with the nurse leaders and other members of the team for mentorship.
